Life Care Management

Our team of Life Care Managers assists individuals to help them remain independent in their home.

Through an evaluation and assessment process, a personal plan is created according to each clients needs and requests. This service is especially beneficial for those with family members who live far away or who find themselves overwhelmed with the demands of daily life.

Life Care Management Services

Advocacy on the Client’s Behalf – Aging life care professionals are strong and effective advocates for clients and their families, promoting the client’s wishes with health care and other providers.

Communication with Long Distance Family Members – It is not uncommon for family members to live out of town and far from their aging loved ones, a Life Care Manager advocates to assess and monitor the situation keeping ongoing communication with the family as requested.

Medical Visit Coordination and Attendance – Attend doctor appointments and facilitate communication between doctor, client, and family; and assist in engaging and monitoring these services.

Household Management – Overseeing and arranging for tasks associated with the organization, financial management and day to day operations of a home.

Emergency Support Services – 24-hour response for Hospital and ER Visits providing Crisis intervention, helping clients navigate through emergency departments, hospitalizations, and rehabilitation stays.

Meet our Team of Life Care Managers

Our team of Life Care Manages bring experience of working with resources in the community and are aware of real-life problems, health and otherwise, that emerge as a person ages. They have extensive knowledge of assisted living communities, rehabilitation and nursing home facilities, and specialized dementia care services.

AnnaMaria Gargano, BSN RN

AnnaMaria started her nursing career in Brooklyn, NY where she worked in the hospital as a labor and delivery nurse for 10 years.  In 2009, after relocating to Florida, AnnaMaria worked for a local hospital in Naples on an inpatient rehabilitation unit as a Charge Nurse.  She received her Bachelor of Science Degree in Nursing in 2018 from Chamberlin University.  AnnaMaria also worked as a home health nurse performing home visits providing skilled nursing services.

AnnaMaria’s acute care hospital experience and clinical nursing assessment skills and knowledge allow her to guide clients and their families in making decisions that ensure quality care.  Her guidance allows for transition planning and problem solving as well as providing a holistic, client-centered approach as clients face ongoing health challenges.

AnnaMaria has six children and in her free time she enjoys reading, working puzzles, cooking and baking.

Laurie Harth, COTA, CMC 

Laurie is a Life Care Manager for Moorings Park Home Care Agency, working in this capacity for over two years. Laurie started her healthcare career when she was in high school working as a nursing technician in the hospital. Shortly after starting her family, she attended Trident Technical College and obtained her Associate of Science Degree and became Certified as an Occupational Therapy Assistant through the National Board for Certification in Occupational Therapy (NBCOT). She graduated in 2009 and started her professional therapy career working in a rehabilitative hospital in Charleston. She specialized in working with ventilator and stroke patients, kidney dialysis patients and patients with mild cognitive impairment.

Laurie was born in Fort Lauderdale and moved with her husband to Charleston, SC in 1997. As much as she loved Charleston, she missed Florida and made the move back to Naples in 2012. She has over twelve years’ experience in the clinical field including direct patient care, supervision, precepting students and care coordination. Her clinical expertise and communication skills along with her compassion and strong commitment to advocacy allows her to assist her clients as they navigate through their healthcare needs.

During her free time Laurie prefers to spend time outside gardening and at the beach. She also enjoys baking and she has a love for all animals.
